Kotoko were handed a huge heartbreak at home after losing 2-1 to ES Setif in the first leg of the CAF Confederation Cup.
The Porcupine Warriors threw away a lead to concede twice in the space of 10minutes.
Kwame Opoku gave Kotoko the lead after 69minutes with a calm finish having received a pass from Latif Anabila.
Substitute El Amin Amoura drew Setif level before Abdul Fedouhli added another to give the visitors the lead.
Naby Keita however missed an opportunity to draw Kotoko level after missing a penalty kick.
Kotoko will now have to win in Algeria convincingly to progress to the Group stage.
